Hip, pelvis or groin pain: when to seek medical help?

This page helps you recognize situations that require medical help before exercising. It does not make a diagnosis and cannot exclude all emergencies. In Luxembourg, call 112 in case of a serious emergency or if you cannot get safe help in an urgent situation.

Three levels of help, without a safety score

An immediate emergency requires action now: call 112 when the situation is serious or seek an urgent assessment with appropriate help according to the instructions received. A same-day assessment is appropriate for some concerning changes that are not accompanied by obvious life-threatening danger. A scheduled consultation allows you to examine persistent symptoms or less urgent limitations.

These categories are not a test that automatically authorizes exercises. The intensity of the pain alone is not enough to decide: a loss of strength or difficulty urinating can count for more than a number. If you are unsure how to interpret a significant change, seek medical guidance rather than waiting to check several boxes.

Understand the picture

Choose the path of help

  1. Now

Immediate danger, worrying neurological symptoms, serious trauma or other emergency: medical help without delay.

  2. The same day

Unusual worrying pain, fever, postoperative problem or severe loss of function: rapid assessment.

  3. Appointment

Persistent pain or limitations: assessment and adapted program, sooner if the situation changes.

These benchmarks do not exclude all emergencies. If there is a significant change or in doubt, seek medical guidance.

After a fall or accident

Significant hip or groin pain after a fall, deformation or inability to bear weight on the leg requires urgent evaluation. Stay in a safe position and don't try to walk or jump to prove nothing is broken. If you cannot move safely, call 112.

Some fractures still allow a few steps. Persistent pain after a fall, particularly with bone fragility, must therefore be evaluated even if you can still put some weight on the leg. If the first image does not show a fracture but suspicion remains strong, the doctor may decide to carry out an additional examination. An exercise does not replace these investigations.

Pain without falling: possible infection or bone problem

A very painful hip with fever, heat, swelling, or altered general condition should be evaluated promptly, the same day. Significant deterioration or discomfort may require urgent help. Do not wait several sessions to see if the pain behaves like tendinitis.

Deep pain that increases with running, then with walking, may require investigation of a stress injury to a bone. The context of increased training or bone fragility matters. Reduce effort and request an evaluation; do not do a repeated jump test. Other bone diseases, including osteonecrosis, require a medical diagnosis and cannot be recognized on a pain map.

Nerve Changes: When to Act Now

With back, buttock, or leg pain, new difficulty starting to urinate, inability to urinate, or an unusual loss of bladder or bowel control may be a sign of emergency. Loss of sensation around the genitals, anus, or between the legs is also a concern.

Significant or progressive weakness, particularly in both legs, requires urgent evaluation. Do not attempt to resolve these changes with a piriformis stretch or pelvic manipulation. Seek medical help immediately; call 112 if the situation requires urgent help or if travel cannot be done safely. The sciatica guide completes these guidelines.

Testicular pain or painful lump in the groin

Sudden or severe testicular pain needs immediate medical assessment, even if it starts after exercise. Some causes need urgent treatment. Do not assume that it is an adductor injury. Do not delay help to try massage or stretching.

A groin lump that becomes painful, firm or no longer goes back in needs urgent assessment, especially if you are vomiting or cannot pass gas or stool. This may be a complication of a hernia. Do not force the lump back in. A less painful lump or ongoing genital symptoms also need a medical appointment, even without these urgent signs.

Pelvic pain, pregnancy and after childbirth

Pelvic pain can come from organs and not just muscles or joints. Urinary, digestive or genital symptoms, unusual bleeding or fever require medical attention. Severe abdominal or pelvic pain with feeling faint, severe dizziness or loss of consciousness may be an emergency.

During pregnancy, contact the maternity ward or the professional caring for you for unusual pain, bleeding or loss of fluid. A reduction in the baby's usual movements requires immediate contact with the maternity ward, without waiting for the next day. After childbirth, fever, heavy bleeding or deterioration in general condition should not be attributed to simple pelvic pain.

In everyday life

Prepare a useful call

  1. Locate

Give your address and explain if the person can travel.

  2. Describe

Specify the change, its onset and associated symptoms.

  3. Follow

Follow the service instructions and keep the telephone available.

Do not delay the call to gather all the information.

After an operation: leg, scar and breathing

Chest pain, sudden shortness of breath, feeling very unwell or loss of consciousness may indicate an emergency, particularly after an operation: call 112. New pain and swelling in a leg or calf requires rapid assessment. Do not try to treat them with a deep massage.

A leaking wound, increasing redness, fever, or unusually severe pain requires prompt contact with the operating team. Sudden pain with the impossibility of moving the hip or bearing weight on the leg requires urgent help. Keep discharge letter and postoperative contact; the instructions specific to your intervention take precedence over the general guide.

A child or adolescent who begins to limp

Limping without an obvious cause should be examined medically. A hip problem can sometimes be felt in the thigh or knee. You should not apply the program of an adult with osteoarthritis to a child or assume that it is simply growing pains.

Sudden pain, inability to bear weight, a deformed leg, fever in a child who seems unwell or worsening requires urgent evaluation. Do not force the child to walk to test. If his condition is serious or if transport is not safe, call 112. The objective is to assess the cause and the course of action to take, without trying to choose a diagnosis with this page.

When to make an appointment even without an apparent emergency?

Persistent or recurring pain that reduces your walking, sleeping, or activities warrants evaluation. Also consult when the chosen measures do not help, a limitation progresses or you do not know how to resume. Nocturnal pain alone does not prove a serious illness; new and continuing pain, with other changes, should be described to the doctor.

Report unexplained weight loss, known cancer, weak bones, prolonged use of certain medications, or inflammatory symptoms. This information does not make a diagnosis, but changes the questions to be examined. The physiotherapist can participate in the orientation; a physiotherapy consultation should not delay necessary medical advice.

What to say during the call and what to do in the meantime?

Explain where you are, what has changed, since when, if there has been a fall or an operation and whether you can put weight on the leg. Mention pregnancy, fever, feeling faint or nerve symptoms. Provide the information requested by the service and follow its instructions; do not hang up until prompted during an emergency call.

Stop exercises that could make the situation worse. Do not drive yourself if your condition or abilities make the journey dangerous. Prepare the usual medications and available documents without delaying help. KinéCheck can guide a non-urgent reading, but does not replace a professional who assesses your symptoms in real time.

Your questions

Is very severe pain always serious?

It can have several causes, but new, sudden severe pain or associated with loss of function must be evaluated according to the context. Conversely, some important signs do not necessarily cause the most intense pain. Don't decide just with a score out of ten.

Can I wait until the next day if the baby's movements decrease?

Contact your maternity unit or midwife immediately, even at night. Do not wait until the next day and do not use a home heartbeat-listening device to reassure yourself. This symptom relates to pregnancy monitoring, not pelvic exercises.

Chapter sources [9]

Does the absence of fever rule out a serious problem?

No. A fracture, certain nerve problems, or urgent testicular pain can occur without fever. No isolated sign nor this list allows everything to be excluded. A significant unusual change warrants seeking medical referral.

Chapter sources [1] [6] [7]

Can I use the office form for an emergency?

No. The form and a callback request are not monitored as an emergency service and do not guarantee an immediate response. In Luxembourg, use 112 for a serious emergency. For another worrying situation, contact an appropriate medical service without waiting for a physiotherapy callback.
