What sensation do you mean?
Some people feel that the room spins. Others feel faint, unsteady or as though they are on a boat. These descriptions are not diagnoses, but help a professional understand the problem.
Explain whether an episode lasts seconds, minutes or remains present, and whether it happens on standing or changing head position. New difficulty walking is especially important to report.

Understand the picture
Balance combines several sources of information
Eyes
They provide information about surroundings and movement.
Inner ear
It detects head movement among other signals.
Body and brain
Signals are combined to guide movement.

How does balance work?
The brain combines information from the eyes, inner ear and body sensors to orient you. Disturbances in these signals or their processing can cause symptoms. Circulation, medicines and other conditions may also contribute.
Head movement challenges this system. It can therefore provoke dizziness even when neck joints are not the cause. A painful neck may also develop because you move less to avoid symptoms.
What is known about cervical dizziness?
This term is used for some combinations of neck pain and dizziness. The Bárány Society, a scientific society specialising in balance, highlights insufficient evidence for a direct causal link between neck pathology and the illusion of spinning.
That does not make your symptoms imaginary. It means other explanations need consideration and claims of treating dizziness by releasing the neck need caution. Relief after massage alone does not prove a cause.

What is benign paroxysmal positional vertigo?
BPPV involves small crystals displaced within part of the inner ear. Certain positions may then provoke brief spinning episodes, such as rolling over in bed. Benign applies to a confirmed diagnosis, not every episode of dizziness in bed.
A clinician uses examination manoeuvres and observes eye movements. When BPPV is confirmed, an appropriate repositioning manoeuvre may be offered. The affected side and canal, and any neck limitations, matter.

What about vestibular migraine?
Migraine can be associated with episodes of vertigo or unsteadiness. Vestibular refers to the balance system. A history of headaches, light sensitivity and other features helps a doctor consider this possibility.
Not every dizzy episode necessarily includes a headache at the same time. Assessment is still required; having had migraine before does not establish this diagnosis.

Which signs need immediate help?
Sudden dizziness with one-sided weakness, speech difficulty, double vision, loss of coordination or new inability to walk may be an emergency. Call 112. Sudden unusual headache or neck pain with neurological signs also needs serious attention.
New severe persistent dizziness warrants prompt medical assessment even without those features. Do not drive while symptomatic. Online balance tests or manoeuvres cannot rule out a serious cause.
What should you record for a consultation?
Describe the sensation, onset, duration, triggers and accompanying symptoms. Mention hearing changes, tinnitus, faintness, an accident and medicines. Explain whether you can walk normally and whether you have fallen.
Depending on the picture, assessment may examine neurological function, balance, eyes or ears. Not everyone needs the same investigations. The aim is to answer a specific clinical question.

Why does treatment depend on diagnosis?
Confirmed BPPV may need a repositioning manoeuvre. Vestibular migraine needs a different approach. Other causes may require medical treatment, medicine review or specific rehabilitation.
Neck care may help associated neck pain without being a proven treatment for every type of dizziness. Check the required expertise and referral options with your clinician; this guide does not confirm specialist vestibular services at the practice.
What can you do while awaiting assessment?
During an episode, get somewhere safe, sit if necessary and avoid situations where a fall could have serious consequences. Ask for help moving around if unsteady. Do not drive or operate dangerous machinery.
Do not repeat rapid movements or repositioning manoeuvres without appropriate diagnosis and instructions. General neck exercises should not be used as a test of whether dizziness comes from the neck.
In everyday life
Describe symptoms before choosing a technique
Which sensation?
Spinning, faintness or unsteadiness.
Which pattern?
Onset, duration and circumstances.
Which other signs?
Walking, hearing, vision, headache and neurological symptoms.
What if symptoms return?
Record changes and the effect of the proposed treatment. Persistent symptoms after apparently suitable care may require confirmation of the diagnosis or assessment for more than one problem. A recurrence does not always have the same cause as a previous episode.
Activity may be resumed gradually after advice, considering fall risk and confidence. Rehabilitation goals and timelines depend on the identified condition; a fixed number of sessions cannot guarantee recovery.
Your questions
If turning my head triggers dizziness, must it come from my neck?
No. Head movement also stimulates the inner ear and can aggravate many balance disorders. The trigger alone does not identify the cause.
Chapter sources [1]
Can I try the Epley manoeuvre myself?
This guide does not provide a manoeuvre without diagnosis. A professional should establish whether BPPV is present, its type and necessary precautions. Self-treatment can then be taught when suitable.
Chapter sources [4]
Does a more relaxed neck mean the dizziness is treated?
Not necessarily. Neck pain and a balance problem may change differently. Monitor both and report persistent symptoms.
Chapter sources [1]
Can I continue neck exercises if I feel dizzy?
Stop if an exercise causes dizziness and seek advice. Unexplained dizziness needs assessment before a general exercise programme.
