When should you call 112?
Call 112 in Luxembourg for chest pressure or discomfort with pain spreading into an arm, the jaw or back, especially with breathlessness, sweating, nausea or faintness. A heart attack does not always follow a textbook pattern. Do not wait for a physiotherapy appointment or drive yourself if an emergency is suspected.
Severe injury, collapse, significant breathing difficulty or a cold and pale hand after trauma also warrants emergency help. The side of shoulder pain alone cannot rule out a serious cause.

Different levels of help
Emergency
Chest symptoms, stroke signs or a serious injury: 112.
Prompt medical assessment
Deformity after injury, new weakness or a hot swollen joint.
Planned assessment
Persistent pain, sleep disruption or limited daily tasks.
What after a fall or impact?
A shoulder that looks out of place, an obvious deformity, severe pain or inability to use the arm needs prompt medical assessment. Fracture and dislocation may coexist. Support the arm without pulling or forcing it, and never attempt to put the joint back yourself.
Visible bone, substantial bleeding, severely stretched skin, or a cold or numb hand increases the urgency: call 112 for a serious injury. A shoulder that returns to place by itself still needs assessment after a traumatic episode.
What does sudden weakness mean?
Sudden difficulty lifting the arm after an injury can involve a tendon tear, fracture or nerve problem and should be assessed promptly. New persistent numbness, progressive weakness or loss of sensation also needs medical advice. Do not repeatedly test with weights.
Sudden weakness in one arm with a drooping face or speech difficulty can indicate a stroke: call 112 immediately, even if symptoms improve. These signs are different from an ordinary painful shoulder movement and should not wait for a routine appointment.
What if the joint is hot and swollen?
A rapidly painful, hot, swollen or red joint may need urgent assessment for infection or another inflammatory cause. Fever, chills or feeling unwell increase concern, but fever can be absent. Do not assume it is just a familiar flare-up.
After surgery, tell the surgical team promptly about an increasingly red or leaking wound, fever or unusual worsening pain. Seek urgent care if you are significantly unwell. Do not massage, heat or exercise a suspected infected joint while delaying medical advice.

What about stiffness in both shoulders?
Marked morning stiffness affecting both shoulders, sometimes the hips, particularly in an older adult, should be discussed with a doctor. Polymyalgia rheumatica is one possible explanation; it cannot be diagnosed from this pattern alone and needs medical treatment rather than exercises alone.
A new headache, scalp tenderness, jaw pain when chewing or visual changes can indicate associated giant cell arteritis. Obtain urgent medical assessment, especially for visual symptoms. Do not wait for the shoulder stiffness to settle first.

Does night pain mean something serious?
Night pain occurs in several common shoulder problems, including tendon pain and frozen shoulder. On its own it does not prove cancer, a tear or an emergency. Position, activity and the overall pattern are relevant.
Nevertheless, persistent worsening pain, unexplained weight loss, fever, a new lump or important medical history such as cancer should be raised with a doctor. Explain the complete picture. Reassuring yourself from one normal movement or becoming alarmed from one symptom can both miss useful context.
In everyday life
Explain what has changed
Onset
State when symptoms started and whether there was an injury.
Associated signs
Mention chest symptoms, fever, weakness or numbness.
Change
Explain what is new, worsening or preventing normal activity.
When is a routine consultation appropriate?
Seek advice when pain worsens, significantly limits the arm, or does not begin to improve after about two weeks. Earlier advice is appropriate when the cause is uncertain or symptoms concern you. Persistent sleep disturbance and loss of everyday function are useful reasons to review the plan.
Prepare the onset, any accident, associated symptoms and difficult tasks. Bring relevant reports and medicines. A clinician can decide whether examination, imaging, treatment or referral is most useful; this page does not assign a diagnosis.
What should you avoid while waiting?
Do not force a stiff or injured shoulder, perform painful self-tests or follow a reduction video. Pause general exercises when urgent signs or an unassessed recent injury are present. Follow existing postoperative or fracture protection instructions.
For an emergency, contact 112 rather than the practice callback form. For other concerns, use an appropriate medical service. KinéCheck only helps navigate body regions; it does not triage symptoms or determine whether waiting is safe.
Your questions
Is left shoulder pain always cardiac?
No, but pain location alone cannot rule out a cardiac cause. Chest discomfort, breathlessness, sweating or faintness requires urgent help; call 112 for a suspected emergency.
Can infection occur without fever?
Yes. A rapidly hot, swollen and painful joint needs urgent assessment even without a high temperature.

Is pain at night a cancer sign?
Night pain alone is nonspecific and common in shoulder conditions. Persistent worsening symptoms and other warning signs should be discussed with a doctor.
