Biceps tendon pain at the shoulder: tendinopathy and tears

The biceps has more than one tendon attachment. Pain at the front of the shoulder, a long-head tear and a tendon tear near the elbow need to be distinguished before choosing treatment.

Which tendons connect the biceps to the shoulder?

The biceps lies at the front of the upper arm. It helps bend the elbow and turn the palm upwards. At its upper end it has two attachments, called the long and short heads. The long-head tendon passes close to the shoulder joint and attaches above its socket.

Tendinopathy describes a painful tendon with reduced load tolerance. A tear means fibres are partly or completely disrupted. Symptoms can overlap, but these situations do not necessarily call for the same treatment.

Understand the picture

The biceps has several attachments

123
  1. Long head

Its tendon passes close to the main shoulder joint.

  2. Short head

Another upper attachment contributes to function.

  3. Arm muscle

Helps bend the elbow and rotate the forearm.

The shoulder attachments are distinct from the tendon near the elbow.

What symptoms can occur?

Pain at the front of the shoulder may increase when lifting the arm, carrying or reaching behind you. Discomfort can extend down the upper arm. A movement may produce a snapping sensation. These features help guide assessment but cannot identify a tendon reliably on their own.

A tear may cause a pop, sudden pain, bruising and a change in muscle shape. A lower bulge in the arm is sometimes called a Popeye deformity. Not every tear is visible, and not all anterior pain comes from the biceps.

Why does the tendon become painful?

Repeated movements, heavier loads and overhead activities can contribute to tendon demands. Tendon structure can also change over time. A sudden injury may cause a tear. One isolated cause cannot always be identified.

The long head is often assessed alongside the cuff, labrum and joint. Focusing only on a tender spot can overlook associated problems. Describe instability, stiffness, injuries and the tasks that have become difficult as well as the painful location.

What does examination involve?

The clinician examines movement, strength and pain location, considering other possible conditions. Some tests load the biceps, but a positive test alone is insufficient proof. Tenderness over the front of the shoulder is not enough to select treatment online.

Ultrasound or MRI can clarify tendons when the result may change care. X-rays show bone and joint information. After a pop or deformity, the doctor also checks whether a tear is near the shoulder or near the elbow.

When is prompt care needed?

Sudden pain with bruising, deformity or weakness after effort or a fall warrants prompt medical advice. A distal biceps tear near the elbow may need different treatment, sometimes surgery. Do not apply advice for a shoulder long-head tear to an elbow injury.

A cold or numb hand, severe injury, a red shoulder with fever or associated chest symptoms needs urgent help. Call 112 for an emergency in Luxembourg. Avoid lifting a weight to test the muscle while waiting for assessment.

What can treatment without surgery offer?

Tendinopathy is often managed through temporary load adjustments, pain relief when appropriate and progressive rehabilitation. A programme considers useful movement, strength and associated shoulder difficulties. It involves more than simply repeating elbow curls.

After a long-head tear, the other upper attachment often preserves substantial function. Some people recover satisfactory activities without repair, although cramps, weakness or discomfort may remain. Assessment helps set realistic goals rather than assuming either complete disability or guaranteed recovery.

In everyday life

Rebuilding a carrying task

  1. Describe

Explain the weight, grip and difficult movement.

  2. Adapt

Reduce the load or bring it closer.

  3. Rebuild

Progress according to strength and clinical instructions.

A progression selected after assessment.

When might surgery be discussed?

Persistent symptoms despite care, or associated injuries needing treatment, can justify specialist advice. Depending on the situation, the surgeon may release the long head, called tenotomy, or attach it elsewhere on bone, called tenodesis.

These are different procedures with different benefits, limitations and recovery rules. Ask about possible effects on cramps, strength and arm appearance. A visible muscle bulge alone does not decide surgery; activities and other injuries also matter.

What precautions apply to exercise?

Initially, the clinician may adjust carrying, overhead movements and efforts that strongly turn the palm upwards. Strengthening is rebuilt according to assessment. Severe pain or lasting worsening after a session calls for changes, rather than repeatedly pushing through.

After tenodesis or repair, follow the prescribed protection protocol. Resisted elbow bending may be temporarily prohibited even when it feels easy. The general mobility exercises on this site do not replace postoperative instructions or clear you to load repaired tissue.

How long can recovery take?

Tendinopathy recovery often develops over weeks or months. After a tear or operation, timing depends on the tendon, procedures and activities required. Low pain does not necessarily mean strength or healing is sufficient for heavy loading.

Follow practical tasks such as carrying a light object, dressing or using a tool. Increasing weakness, disabling cramps or lack of progress deserves reassessment. An image or an elapsed number of weeks cannot describe the entire outcome.

How can return to activity be prepared?

Specify whether the task involves carrying palm-up, pushing, pulling, throwing or overhead work. These actions load the shoulder and elbow differently. They help the physiotherapist build stages relevant to your life.

Increase one difficulty at a time and use the response afterwards to guide adjustments. Keep the operation report and movement or loading restrictions available when relevant. Rehabilitation must account for all treated structures, not just the word biceps in a diagnosis.

Your questions

Does front-of-shoulder pain prove biceps tendinopathy?

No. Several nearby structures can cause similar pain. Examination considers the whole shoulder and the circumstances.

Chapter sources [1]

Does a Popeye bulge always need surgery?

No. Function, cramps, symptoms, activity needs and associated injuries influence the decision after assessment.

Chapter sources [2]

Is an elbow tendon tear the same problem?

No. A distal tear near the elbow needs its own prompt assessment and may have different surgical indications.

Chapter sources [2]

Can I do curls after tenodesis?

Only when your protocol permits resisted elbow bending. An easy or painless movement can still load a healing repair.
